Yes, unfortunately modules doesn't work as expected using drush.

Patch #9 hasn't solved the issue.

Patch #5 works well.

I've updated the solution from #66 and create a merge request (attached patch is the latest version of the changes in MR):
- cleaned up a code a little bit
- made the create_new_items setting processable in js
- implemented dependency injection
- fixed langcode handling for term creation.

Not sure it is relevant anymore. Based on my tests blocks with max-age = 0 are placeholdered in Drupal 9.5. Or we need to add test to show the use case of the issue.
