Installer does not warn the user that cookies must be enabled with the correct cookie domain (and fails when they aren't)

When using install.php with cookies disabled, you get mysql errors which seem to halt the database population. The installer should not rely on cookies, and if it has to, it should warn the user that cookies must be enabled.
