Managing comment fields and field displays is missing features

Out of the box, comments list/capture the following fields:

But mange display only lists:

As a result, I cannot:
- Collect author, but choose not do display it
- Collect title, but choose not to display it, which results in redundant displays like: https://skitch.com/jeff.noyes/8ca64/i-have-lots-of-wysiwyg-data-drupal-g...
- Change the display of a comment to be different from my input
- Etc.

Additionally, if I add the module 'comment notify' it doesn't show up as a fiel:
- showing the module is enabled and my intentions:

I raise these questions because we've had clients turn their noses up over Drupal's comment display. In their minds, this is an integral part of building communities. Here's what many customers experience as Drupal's commenting:

When in reality, what they want is this:

To make this happen, we need to allow site builders the opportunity to move contributed comment fields, as well as manage their display. Out of the box, it would be great if Dupal shipped with a solution like:

    Makes Drupal easier to use. Preferred over UX, D7UX, etc.
