I've found it! Module Filter β is to blame.
I'm using Drupal 10.2.6 and PHP 8.2.12. Gin 3.x-dev@dev.
Sorry, but testing this on the latest 3.x-dev@dev, I see two header rows on the permissions table (one of which is sticky) and no means of scrolling horizontally to roles that overflow the right of the container.
I can confirm that 1.3.2 resolves the issues. Many thanks for your support.

Filtered field type: Entity Reference to entity types created using the ECK module
So probably something to do with ECK compatibility!

I can confirm that this is still an issue as of D10.1.7. Patch #44 applies cleanly and resolves the issue for us.
