Thanks @Avvion, it would be interesting to hear, if this still happens with 3.x and Captcha 2.x, which received several bugfixes. Are you planning to upgrade soon?

Is it possible for you to upgrade to Drupal 10 and use Captcha "2.0.0-beta3" together with Riddler "3.0.0"? This seems like a very unusual issue. I will have a look.

You could also try only using "Mainz" as an answer and enable the case insensitivity setting.

I think we just found the problem, through creating tests in ✨ Add tests to ensure case-unsensitivity works Fixed . We recently had a customer with a very similar issue, only your screenshotted log message seems to be a bit off, but maybe it will help!
