Display section name on access scheme forms

Problem/Motivation

When adding editors or roles to access schemes, only the scheme label is displayed on the form making it difficult for admins to know what section they're adding to.

Steps to reproduce

Create workbench access scheme from admin/config/workflow/workbench_access
Select 'Sections' for the created scheme
Add editors or roles to any of the sections - only the scheme label is visible on the form.

Proposed resolution

Modify page title callback to display both the scheme label and section name for entity.access_scheme.by_role & entity.access_scheme.by_user routes.

i.e.
Editors assigned to %label for %section
Roles assigned to %label for %section
