The issue is occurring due to use of pager in DB query while getting all the bundles. I have replaced the pager with range function and it is working as expected for me.

Steps to reproduce
- Install "Admin Toolbar Extra Tools"(admin_toolbar_tools) module.
- Go to the following path and configure the form like the below screenshot.
- Hover through the following menu
- Structure > Content types
- Structure > Custom block types
- Structure > Media types
- Structure > Taxonomy
