[policy] Document Libricon semantics, including examples

Problem/Motivation

We don't provide clear guidance one when to use Libricons and what they mean. This may lead to them being used inconsistency in core and contrib.

Proposed resolution

Document the standards on https://drupal.org/ui-standards

1. Agree on the standard in this issue.
2. Update https://drupal.org/ui-standards with this new standard.
