On below issues some discussion done for node saving by programming.

There triggered alias parameter when node save.

I have checked below options :

1) Generate a URL alias for un-aliased paths only
2) Update the URL alias for paths having an old URL alias
3) Regenerate URL aliases for all paths

All time generated path aliases.

Something missing in pathauto configuration or you can provide steps for re-produce issues.
