Improve documentation of entity_lookup for specific entity types

Problem/Motivation

Certain entity types have non-standard field names, which can result in unexpected data from the migration.

Proposed resolution

Add extra documentation to entity_lookup to note when entity types have specific requirements on the lookup settings.
