Blocked users continue to receive notifications

After blockning user their still have notification from subscribed contents.

- Subscribe a user in some forum/topic
- Disabled the user account
- Add content to these forums/topics

#1 Delete subscriptions when disabling a user
#2 Check if a user blocked before adding them for notification
