Update README to follow two-step process of new fields

Problem/Motivation

The "add new field" process is happening in many steps. In newer versions of Drupal you first select the Type (ie: Reference) and then the option, instead of having a dropdown with all the options.
Documentation is for the old way of adding fields.

Steps to reproduce

Go to the add new field, you will not see a "Display Mode" entry.
