Library settings should not be wrapped in fieldset

The settings form for accessible menu has a fieldset for "Library settings" and then generates fieldsets inside of that for each library it finds.

The fieldset for each library is fine, but the wrapper "Library settings" fieldset shouldn't be a thing. It should just be a heading.
