When a user or a role is deleted, cleanup the node_access tables

When a user is deleted, let's also cleanup the node_access table.

When a role is deleted, let's clean up the node_access table and our table mapping rids to node_access gids.

@joey-santiago I am unable to reproduce this issue because the rows in the node_access table were deleted when the role and user were deleted and i think it is working well because hooks written in .module files working fine.
Can you please check and give more details if i missed something.
