Form does not show multiple items

Steps to reproduce

Create a custom field with several subfields. Make sure it is multiple.
Create an entity that contain at least two instances of the custom field
Edit the content type. It shows only the first instance.

    Sorry but there is no way I can possibly debug your issue as described. I'm using this for various cases of multi-value instances as well as other users. You have something else going on I assume that needs to be explained.
