Consider making a reference site and thinking over what is different about the setup of the problematic site.
I've created a new site with Drupal 11.0.1
I am using Media and Media library for images
Also, using ckEditor5
When I add an image to a node the image shows the file name below the image.
When looking at the Grid view in Media it shows the file name below the image.
I have spent way too much time trying to remove the file name.
This is probably right in front of me but I am missing it.
