I've just come here to say exactly the same thing :)

The UI shows example structure for the required CSV file, but if you name your columns as specified, import fails.
The UI must match the correct values which do appear in the error message.
See screenshot.
go to /admin/config/search/redirect/migrate
Create a CSV with the header names specified in the UI, e.g. `Status Code`, as opposed to `status_code`
Change UI text to match actual required header values.
