Allow promoted result to link to external url

Problem/Motivation

Promoted results are currently created by specifying trigger search terms and making a reference to a content node for those trigger words.

Sometimes, rather than have a promoted result tied to content on the site, a promoted result should link to a url. This is useful when there are portions of a website domain that are not part of the drupal site.

Proposed resolution

Add a radio button to select between internal reference and external url. Provide two different fields depending on the radio button.

Data model changes

The entity that represents a promotion will need another field attached to store a url.
