QR does not show

The QR code is working fine. Tried in Drupal 10.2 and module version 8.x-1.0.

I used: composer require 'drupal/one_time_password:^1.0'

I solved the issue, it was my system protection configuration.

The error persist, even in 9.5, now I have more information, showed in the attached image.

Now works! I updated the Core and my Modules.

Glad you resolved the issue. Please note Drupal 9.5 is unsupported.
