Add Country/City to Tenant launch form

@yas, please review this patch. This adds two things

1. On the OpenStackProviderAdminSettingsForm form, it adds country/city/zoom level fields for the administrator to select default values.

2. On the Tenant creation form, the tenant user now has the option to change the country/city and zoom level specified by the administrator. They can choose a different country altogether.
