Remove usage of environment variables ($_ENV)

Problem/Motivation

We are not using environment variables any more. We should use the values from settings config.

Proposed resolution

  1. Replace usage of $_ENV variables with values from config
  2. For items the “MASTER_DOMAIN”, create a new form element in Settings form in the default proxy host settings group, save it and use it appropriately
