Fatal Error: missing arguments when using remove instance form

When using the code updates in 🐛 Error: Non-static method SmartDateRecurrenceFormatter::augmentOutput() cannot be called statically Fixed there is a fatal error when submitting the instance remove form, either with or without AJAX.

Update the form code to use dependency injection and then pass the required arguments in when creating the Instances controller.
