Form in custom block does not render page correctly in submission results

I will recreate the issue and also try to fix it and provide the patch file for the issue.

I recreated the issue as per mentioned steps.
and can replace the block URL(ending with block id) with the form or page id as needed but,
it is rendering from file "WebformSubmissionListBuilder" from "buildRow()" where data[] array
containing details related to the custom block we had created.

So, for solving this issue do we only need to change the URL of the Submitted to Column
OR
We need to change the whole data inside the data[] from block to page specific.

kindly suggest the approach.

it turns out the second option, as you indicated

We need to change the whole data inside the data[] from block to page specific.

If you don't want the custom block to be the source entity, you can uncheck "Use this field's Content entity as the webform submission's source entity." via the webform field's display widget.
