List form does not reflect user's subscriptions

Problem/Motivation

When showing the subscribe form block, the checkboxes should indicate whether the user is signed up to each list.

Steps to reproduce

Add the block
Subscribe to a list
View the block - all checkboxes are unticked

Proposed resolution

Refactor CampaignMonitorUserManager so that it is part of the main module
Use getUserSubscriptions to populate the default values in the block.
