I'm not sure if this is a bug or working as designed. If it's an intended behaviour then I would mark this as a feature request with an option from usage settings to turn this on.
When a revisionable entity has usage for the current item from multiple different fields, the list doesn't show all the fields but instead just skips after the first field.
For an example, if a node N item uses a media M on entity reference field A, entity reference field B and entity reference field C (that are not part of paragraph items or a collection field), the entity usage list from Media M just lists one entry of node N which is linked to field A only but completely skips field B and field C. With that, if the default revision has the usage for that media from field B and field C, then field B only gets shown and field C is skipped completely.
This is a confusing behaviour for content editors and not precise stats wise for the usage list.
Show separate entries for different fields for the same content item or concatenate the field names by comma or some delimiter from the same row of the content item.
