Accessibility: Remove outline for :focus

Problem/Motivation

Elements should not be using designed focus styles at this point in order for focus styles to be considered accessible.

Remove the default :focus styles for outline color, width, style etc in the CSS

This is considered complete once the focus style for interactive elements reflects the browser you are on (white/blue outline for Chrome, black/white dashed for Firefox, blue outline for Safari)
