Migrate Countries module field

Not sure, it would be easier to provide plugin out of the box instead to find a plugin somewhere else. Once it will be commited it will provide more feedback from the developers who busy with migration.

I already forgot about this issue as I finished migration year ago without issues by using changes from MR.
