Retaking a failed quiz still has the previous question index

Problem/Motivation

Notice: Undefined offset: 11 in quiz_take_question() (line 1901 of quiz.module).

I was noticing these in my logs, and while trying to reproduce it locally I was able to by failing the 10 question quiz first then trying to retake it.

Proposed resolution

Reset the index if it doesn't allow resuming.
