The email field is still shown

Hide the email field from the simple password reset form.

Can confirm the patch works as expected.

1. Drupal 7.78
2. simple password reset 7.x-1.6
3. Create a new user
4. With this new user, request a new password
5. Click the link in the new password email

Expected: Email address field hidden
