Address field is JavaScript only

Currently this field relies entirely upon JavaScript to work. When an option from the country select list is chosen, the rest of the fields in the form are rendered with AJAX. However, when JavaScript is disabled the field does not work whatsoever as only the country select appears.

I understand the motivation and design of the widget in this way but there must be at least a rough fallback we can provide to non-JS users. This is made much easier in cases where the widget provides only a single country to choose from. I will be able to provide a patch when this is discussed and agreed upon.
