A suggestion from benjifisher in Slack - use the entity destination plugin, like Menu and Block use.

Changing the migration plugin to call them "defaults", and using the entity destination plugin.

Bumping to the v2 branch.

The merge request is now down to just needing to finish the new tests.

I've finished this, it's all working now.

Need to see if there are any additional tweaks to make, but otherwise it's ready for 2.1.0.
