Table of contents in README.md should use links

Our README.md file has a table of contents at the start, but the entries don’t link to the corresponding headings. We should amend that. (See here and here.)

Note that the README.md template → for some reason doesn’t specify that the ToC should have links, but that seems more like an oversight. I don’t think there’s a proper reason for that.
