add config setting for example postcode in form UI text

Problem/Motivation

Currently, the form hardcodes UI text which contains an example postcode.

This is hardcoded to LA9 4DQ, which is the Kendal office of Westmorland and Furness council, so not ideal for councils around the country, which may want a local postcode.
