Allow configuring required status of title and published_as fields

Problem/Motivation

The title and published_as fields are both base fields so their parameters cannot be edited like fields added to the review type by the site builder. Some site builders may want these fields to be required to be filled in and will have to write a form alter hook to do this.

Steps to reproduce

Proposed resolution

Provide configuration checkboxes in the review type to control if these fields are required. Read configuration when building the review form to present to a customer.
