node_access_rebuild_progressive: Integrity constraint violation

I have about 2 million entries to recalculate, and after 700K I get following error in drush output:

SQLSTATE[23000]: Integrity constraint violation: 1062 Duplicate entry '1979389-3-nodeaccess_role-nl' for key node_access.PRIMARY

It's on a production system, as far as I know the database is in a correct state. I also ran it during the night so there's little chance someone was changing data manually.

    We also get the same error on a website with a lot of nodes.
    I have not confirmed this but my theory is that it happens when the Drush and cron run at the same time.
