Add Title/Description to Config Page

Problem/Motivation

When installing the module, there's an expectation to configure it and put in the contents yourself. It would be great to split up the textfield configuration a bit to make it easier for users to enter the data.

Steps to reproduce

Install the module
See that "optional details" appears in your llms.txt.

Proposed resolution

Rather than seeding the config in the install process, have config entries specified so that the site can build the llms config dynamically.

There is now a title and description field.

The llms.txt file has changed a bit.

Got an idea for the upgrade path: Add a checkbox for title and description which toggles whether or not they're displayed. Default to ON.

Added toggles for the TITLE and DESCRIPTION to make it backward compatible, and allows you to manually provide the file if you really want to.
