Clean up date formatting on user subscription page

Problem/Motivation

The "last known state" message on the user subscription page contains unwanted markup, and the date format is not set. There is also an unused "%name" placeholder.

Proposed resolution

Change the "%time" placeholder to "@time", add the default "long" format to the date.formatter service call, and remove the unused "%name" placeholder.
