Created Forms not Showing

After creating a fillpdf form, nothing is showing in the list at /admin/structure/fillpdf.

The file is being created successfully, i can see it in the private files directory, but nothing is showing in the list at /admin/structure/fillpdf.

The file is visible in the Entity Reference View I've created, and the file can be used by fillpdf to create a final pdf file.
