Duplicate file uploads

Duplicate files are uploaded when using File Browser with a media field in a paragraph.

Create a paragraph type that has a media field (ours was for Documents)
Add a paragraph entity reference field to a content type and configure it to allow/use the new paragraph type
Edit a node and add a new paragraph in the field in step 2.
Click on Add Media in the paragraph media field
Click on the "Open File Browser" link in the Add or Select Media dialog
Click Upload in the File browser window and upload a file of the type required by the paragraph media field
In a separate window/tab, navigate to Content -> Files and you should see the new file has been uploaded
Back in the File Browser window, double-click on the newly uploaded file in the file list. The window closes and you will be returned to the Add or Select Media dialog where you can continue to attach the file. Don't click on anything yet.
Return to the window/tab displaying Content -> Files and click on the browser refresh button
You will see a duplicate file has been uploaded

It's probably the field widget that is duplicating the file as IMCE does not have such a feature.
Not sure which module is responsible in your case, paragraphs or media, you may consider moving the issue to the related project.
