AX: Inconsistant use of labels or instructions.

Problem/Motivation

Out of a AX-scan there came following points:

• input fields with hidden labels should have an aria-label attr.
• We only want alt attributes on images.
• Remove title attribute form required label in _form-label.twig.

Proposed resolution
