Embed Content accordions removed when saving modal

When adding multiple embed content accordions, after initially saving the accordions and then editing again and clicking "embed", all accordion items except the first one are removed. Screenshots below demonstrate the behavior.

1) Add multiple items, click Embed button

2) Multiple items saved successfully

3) Edit items, click Embed button

4) Only first item remains
