Add support for Attribute-based plugins

Problem/Motivation

Currently the form_decorator supports annotation plugins for form decoration only.

Proposed resolution

Add support for Attribute-based plugins. And update all documentation accordingly. It seems core and other modules continue to support annotation plugins - so I'd do the same for this module.
