Undefined array key "suffix" when using Integer field

I added a field to my new drupal install with this module. When I did a setup with a string and an int, it threw an error.

Steps to reproduce

1. Create a new data field.
2. Set type to Number (integer)
3. Attempt to add a prefix/suffix
4. System doesn't crash but on the edit content type, the error message is displayed
