Does this make a menu entirely manual or can parts stay automatic?

This module changes the menu used for filling the admin toolbar. Apart from that it works like any other menu. You can add menu links automatically to menu's using derivers, you can read more about that in this article. The Admin Toolbar Content module → might also do what you're looking for.
