Existing file from another media entity overwritten

Problem/Motivation

Existing file from another media entity is overwritten if the the newly uploaded file via the edit form has the same name.

Steps to reproduce

1. Have an existing media item "Media A" with image file name "myfile.jpg"
2. Use the edit dialog on another media item "Media B". Replace the file with "myfile.jpg" (different file but same name)
3. "myfile.jpg" from Media A is overwritten with "myfile.jpg" from the Media B.

Proposed resolution

I would expect Media B to have renamed the file as there was an existing one already.
