[ai moderation] Settings form allows infinite models to be added

Problem/Motivation

The settings form for the AI Moderation module has a submit button labeled "Model (0)" that when you click it adds an additional dropdown to choose a model. This seems to be to allow you to specify the use of more than one model in a specific order. However, the labelling is not clear, and in cases where the Provider only has one usable model (as with Open AI) it is still possible to click the button an infinite number of times.

Proposed resolution

The button should be relabelled to make it clear what pressing it does, and it should only appear if it is possible to add more models for the selected provider.
