Default approval permissions too permissive

I thought this would be an easy fix. But it isn't turning out to be that easy. I switched around the default permission. That didn't work. Then I dismissed the request. That made things worse. Now I can't see the approval, nor can I re-add a new approval with that person since we have a no-duplicates policy.

GitLab is taking forever to complete the merge train, so technically not fixed yet
