I have reproduced this issue on user and node entity it works fine,
The provinces change to States. Here are the supporting screenshots.

To be clear, you are saying it is an issue on user entities, but not an issue on node entities?

It works fine for both the entities.

OK, I'm having an issue on a custom fieldable entity. Not sure if there is something about the entity itself or that the field just doesn't work with custom entities.

We support custom entities just fine (profiles, commerce stores, etc). Of course, it depends on how you've implemented your own form, as there could be bugs there. Check the dev console in your browser, check watchdog, see if there's an error you can investigate.
