Required score on unevaluated behavior

Problem/Motivation

Currently we always allow students to proceed to the next lesson if there are unevaluated answers in the current one, even if the score they'll get with max possible on evaluation will still make them fail.

Should we add some settings on lessons to:

1. Control if a failed required lesson should be restarted or not (if a single required lesson is failed but with revisit mode students can always go back to the course and correct their answers)
2. Control max allowed number of retries (a new integer take_count field will be needed on lesson status entity and allowed number of retries on course lessons item data where 0 is unlimited for exam mode)
