Title and link in toolbar do not update

@dpi I am not able to replicate this issue, Please help me to review this. Needs more info to test this.

Re-visited this today since the patch no longer applies to 1.1.0, after updating from 1.0.0

Steps:

1. Visit an existing node
2. Edit node, change title.
3. View node again.
4. Notice: the Edit TITLENAME link at top left of page still has old title.

Conflicted due to 🐛 Edit link in breadcrumb region shows even when user has no permission Needs review .
